   The banana was eaten.

Active:  The instructor lost his lecture notes.
Passive: The lecture notes were lost by the instructor.

Tough choice here.  It’s probably better that the instructor keep his notes.

Active: My niece decorated the birthday cake.
Passive: The cake was decorated by my niece.

Keep it in the family and emphasize the niece.
